- Help shape a new care delivery model serve as the clinical lead and collaborate closely with nephrologists, dialysis staff, and care coordination teams
- Build meaningful long-term relationships with a stable, consistent patient panel
Responsibilities
- Provide comprehensive primary care services to adult patients with chronic kidney disease and comorbidities
- Average patient visits per day, focusing on prevention, chronic disease management, and coordination of care
- Conduct annual health evaluations, medication management, and care plan development
- Collaborate with nephrologists, dialysis teams, and other providers to reduce hospitalizations and improve health outcomes
- Participate in care redesign feedback to optimize the program s clinical model
Compensation & Benefits
- Competitive base salary: $130,000 $170,000
- Productivity bonus tied to quality outcomes and care metrics (up to 60% of base)

Candidate Requirements
- Minimum 5 years of experience as a nurse practitioner in primary care, internal medicine, or gerontology
- Licensed to practice independently in Virginia
- Active DEA license and experience managing chronic conditions such as diabetes, hypertension, COPD, CHF, and hyperlipidemia
- Experience in dialysis or nephrology a plus
- Proven ability to work independently while collaborating effectively with a broader care team
- Strong interest in value-based care and improving outcomes in vulnerable patient populations
