Position Highlights:
Deliver primary and urgent care in patients homes or via telehealth
Work with a supportive, multidisciplinary team focused on patient-centered outcomes
Manage chronic conditions and provide holistic, high-touch care
Participate in care transitions, social determinant interventions, and complex care coordination
Ongoing clinical education and support from experts in geriatrics, psychiatry, pharmacy, and palliative care
Schedule:
Flexible care delivery (home visits & virtual)
After-hours call coverage rotation required
Requirements:
Board-certified NP or PA with active Washington license
Active DEA and BLS certification
At least 5 years of licensed driving history with clean driving record
Must be able to travel within region as needed
