Job Function:
Provide office-based management for a geriatric patient population.
Promote and implement geriatric care protocols and standards across all adult medicine providers within the network.
Educate Internal Medicine residents on the principles of geriatric care, including through lectures and clinical teaching.
Develop care transition models to support home health, family involvement, and post-acute care planning.
Collaborate with Hospital Medicine and Emergency Department physicians to establish core competencies in managing the unique needs of geriatric patients.
Create referral protocols to specialists commonly involved in the care of older adults, such as rheumatology, endocrinology, and cardiology.
Job Requirements:
Board Eligible or Board Certified in Family Medicine or Internal Medicine, with completion of a Geriatric Fellowship.
Active New York State medical license required.
Current ACLS or BLS certification required.
