Geriatric Care is seeking an experienced Nurse Practitioner to join our team in Evansville, IN. This role focuses on providing comprehensive primary care to participants in the PACE (Program for All-Inclusive Care for the Elderly) program. You will work under the supervision of the Medical Director/PCP and manage complex clinical situations for patients aged 55+.
Key Responsibilities- Deliver direct and indirect care in the PACE Health Clinic and Day Center.
- Manage and treat complex comorbidities in a frail, elderly population.
- Conduct assessments, follow-ups, sick visits, and occasional home/facility visits (1 3 times/week).
- Collaborate with interdisciplinary teams to ensure holistic patient care.
Schedule
- Mon Fri: 8:00 AM 4:30 PM (30-minute lunch)
- On-Call: Phone-only; rotated with 2 other providers, including weekends (4:30 PM 8:00 AM and all day on weekends).
- Weekly Compensation
- Pay Rate: (higher rates require strong NP experience)
- On-Call Rates: Flat Rate

- Board Certified through ANCC or AANP
- DEA (Indiana)
- BLS
- Licensure: Indiana NP license
- Experience:
- 3 5 years RN experience preferred
- 1 2 years NP experience REQUIRED in primary care
- Experience with elderly population and complex comorbidities strongly preferred
ASAP (target 2 3 weeks after offer)
Patient Volume- Average 5 6 patients/day (variable based on assessments and visits)
- Current program size: 71 participants (may grow to 100)
