The RN Case Manager - Home Care provides in-home nursing assessments, care coordination, and patient education to support health management and safety for patients in their own residences. This role requires a registered nurse license, CPR certification, and experience in nursing and case management, with responsibilities including wound care, medication management, and interdisciplinary team collaboration. The position offers flexible scheduling, mileage reimbursement, benefits, and focuses on improving patient quality of life through personalized home care services.
Registered Nurse (RN) Case Manager - Home Care: Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The RN Case Manager would provide head to toe assessments, care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.

Territory : This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position of where the patients live would be the southeast section of the Twin Cities Metro area.

- Annual Salary Range: $89,419 - $106,620

- Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the State of MN.
- Able to travel within a geographic area serviced by the office. Reliable transportation, valid drivers license, and proof of auto insurance is required.
- CPR Certification is required
- Two (2) years of current nursing experience preferred.
- One (1) year case management experience preferred.
- Preferred: Community health and a wide range of Medical-Surgical experience.
- Perform assessments and procedures, which require knowledge, judgment, and nursing skill based upon principles of psychological, biological, physical and social sciences.
- Visit in home clients to evaluate the quality of care being provided and review clinical notes.
- Participate in ongoing interdisciplinary assessment of the patient.
- Provides services that are ordered by the physician as indicated in the plan of care.
- Counsels and educates the patient/client and family in meeting healthcare related needs.
- Completes timely and accurate clinical notes including addressing the patient’s/client’s progress.
- Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge.
