Responsibilities:We are looking for a Registered Nurse (RN) Navigator to join our Neuro team at our downtown Fresno campus. In your role, you will be responsible for ensuring that assigned patient populations receive high-quality and comprehensive services. As part of a multidisciplinary team, you will act as a patient advocate, educator and coordinator, facilitating continuity of care with the healthcare team. You will also serve as a liaison throughout the hospital system and in the community, promoting the services provided to our population.

Qualifications:Education & Experience
- High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
 
- 3 years of recent nursing experience in the area specialty required
 
Licenses and Certifications
- RN - Current and valid Registered Nurse license to work within the state of California required
 
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
 
- NATIONAL - National Certification related to specialty and applicable to role OR Bachelor of Science in Nursing (BSN) Degree required
