A growing post-acute care organization is seeking a compassionate, mission-driven Physician to join its expanding clinical team. This role focuses on providing high-quality primary care to residents in long-term care settings, supporting a healthier, more dignified aging experience.
About the RoleIn this position, you will provide comprehensive primary care to adults and seniors residing in long-term care communities. You ll work in a supportive, physician-focused environment that prioritizes clinical quality, collaboration, and work-life balance.
This is an ideal role for experienced physicians or recent graduates who want to practice meaningful medicine with predictable daytime hours and strong organizational support.
ResponsibilitiesConduct admissions, evaluations, routine assessments, and treatment plans
Manage chronic and acute medical conditions in the LTC environment
Collaborate closely with interdisciplinary care teams including NPs, nurses, therapists, and facility partners
Communicate with families and caregivers to support continuity of care
Document encounters accurately using a provided mobile device
Provide compassionate, patient-centered care with a focus on dignity and quality of life
Monday Friday, daytime hours only
Minimal on-call responsibilities
No nights, weekends, or holiday rounding

$225,000 $300,000 salary

Board Eligible or Board Certified Internal Medicine or Family Medicine Physician
